New Patients

We are currently accepting enrolments for new patients.

If you wish to register with Mercury Bay Medical Centre as your main provider for your medical services and you live in New Zealand for longer than 6 months (183 days) of the year, please download our enrolment form and eligibility guide below:

Please include two forms of ID, one must be photo ID.

Please note, enrolling at our practice will remove your funding from your previous practice and any publicly funded care outside of the Waikato DHB catchment.

You are also welcome to collect an enrolment form directly from reception.

Please note you may be charged as a casual patient for your first appointment if we are unable to complete your enrolment in advance of your appointment.
